#Testing Payment Plan Form
Please Complete the below application to submit a request for a payment plan. Your application will be submitted for review and a reply will be sent to you at the email address provided in the application. If approved, the payment plan will be included in the reply email and you have 3-days to return the signed agreement or it will be invalid. The following information will be identified for review of your account:
- Property address and Commission account number
- Water/Sewer account status (account must be active)
- Prior Payment Plan Default within the last 24-months (accounts that have had a default within 24-months do not qualify for a payment plan)
- Current fiscal year Low Income Heating Home Energy Assistance Program (LIHEAP/Fuel Assistance) approval letter. If you do not have LIHEAP approval, you may still qualify for a payment plan.
